Create a form

1. In the Creator, click on CREATE, select Form, and click on Create Form from scratch.
2. Click on ADD A BLOCK and select one of the blocks.
3. Complete your form creation using any combination of form blocks.
4. Under Results, edit the result block(s). Go to the Result blocks help guide.
5. Publish your form.
6. Embed your form.
