Create a form

Collect emails, names, and other information from your audience using a standalone form.

1. In the Qzzr Creator, click on CREATE Q, select Form, and click on Create Form from scratch.
   
   
   You can also select Create Form from template to start with a pre-built form.
2. Click on ADD A BLOCK and select one of the available form content blocks.
   
3. Complete your form creation using any combination of form blocks. You can also customize form fields.
   
4. Under Results, edit the result block(s). Go to the Result blocks help guide.
   
5. Publish your form.
   
6. Embed your form.
